Removing broken locking bolts

چکیده
Removal of broken locking bolts requires careful planning. The following tips facilitate the procedure and reduce operation time. Positioning the patient supine without traction allows optimal adduction. Early screw removal will leave the nail rotationally unstable; apply the jig beforehand. If direct palpation is not possible, access the end cap by a guided drill (Fig 1). Use screw holders (Fig 2). Retract the nail to realign the broken screw with the nail hole. Tap the fractured screw to advance it into the far cortex for purchase (Fig 3). Screws can then be removed with a broken screw set (Fig 4).

Removal of locking plates: new implant, new challenges and new solutions
Removal of locking plates in many ways poses novel challenges compared to conventional plates. None of the techniques described for the removal of locking plates are adequate for all situations. We report our experience of 27 patients from whom a total of 33 locking plates were removed. We also describe a novel technique for the removal of locking plates which in our experience could be used in...
متن کاملImpact of posterior tibial nail malpositioning on iatrogenic injuries by distal medio-lateral interlocking screws. A cadaveric study on plastinated specimens.
In intramedullary tibial nailing, multi-planar locking makes stabilization of proximal and distal metaphyseal fractures possible. A known complication in intramedullary nailing of the tibia is iatrogenic injury to neuro-vascular structures caused by the insertion of locking screws. As shown in previous studies, the distal positioning of the nail is important, as it determines the course of the ...

satisfactory functional level, which may be crucial to elderly patients? Addressing these issues, various locking screws and implants are rapidly gaining popularity for these patients in certain regions. Such implants come with screws that directly engage the plate to create fixedangle bolts, thus offering better fixation in osteoporotic bone. Volar application of such locking implants can be u...
متن کاملIntramedullary fixation of proximal humerus fractures: do locking bolts endanger the axillary nerve or the ascending branch of the anterior circumflex artery? A cadaveric study
BACKGROUND Proximal humerus fractures are one of the most common fractures. Intramedullary locked nailing is becoming a popular alternative treatment, especially for easier fracture patterns. Although axillary nerve injury has been reported, no study has compared the safety of the proximal locking options relative to the axillary nerve and the ascending branch of the anterior circumflex artery....
متن کاملRemoval of a broken intramedullary femoral nail with an unusual pattern of breakage: a case report
To the best of our knowledge, only 3 cases, including the present case, have been reported with a three part broken pattern. However, this is the first case associated with a distal locking screw broken. We report the case of a 31-year-old patient who sustained an open femoral shaft fracture . The fracture was stabilized with a Kuntcher femoral nail. After 7 months of the initial surgery he pre...
